Student Forms
Pennsylvania Code for Child Day Care Centers and the Association for Christian Schools International (ACSI) require certain forms be held in a student file at the preschool facility. The following forms are what Calvary Preschool require for each student at the time of enrollment:
Student Emergency Information (PDF Document)
Child Information Sheet (PDF Document)
Health Profile (PDF Document)
Allergy Profile -- use only if your child has allergies that would need treatment
Physician's Health Assessment (PDF Document) (required 60 days after the start of school)
Immunization Records
Custody or Protection From Abuse (PFA) Letter (PDF Document)
Picture Release (PDF Document)
Parent's Statement of Understanding -- not yet available (PDF Document)
*** According to HIPPA standards, all medical information is kept confidential and locked in a filing cabinet in the Preschool office. Information is only released with parental consent and a signed waiver.
Every year after initial enrollment, the Student Emergency Information,Custody or PFA Letter, Picture Release, and Parent's Statement of Understanding need to be refiled. If changes occur in the Health Profile and/or the Allergy Profile they will also have to be updated and refiled.
